Venue: Hospital Sultan Ismail
8 am - 8:38 am, went to Klinik Kesihatan Sultan Ismail
- go to Klinik Demam, room 9
- take number from staff
- pass the ic to staff and register
- pay RM 1 by using credit/debit card (clinic don't accept cash)
- see clinic doctor
- let the clinic doctor issued refer letter to eye specialist at HSI
Then, walk to Hospital Sultan Ismail (HSI)
- go down to canteen
- go to ophthalmology clinic (eye) , which located at Level 2, to see eye specialist
- After see eye specialist, go up to level 3, take number to register for making appointment card
- Go down to level 2, specialist ophthalmology clinic (eye) counter to make appointment for next time
- Go up to level 3 again, to get medicine from HSI pharmacy
Record:
On 3 January 2023 (Tuesday) night, after dinner, bring my wife to see doctor at Klinik Family Care at Taman Adda. Doctor issued her mc and refer letter to hospital. But the doctor told us that my wife still need to go to klinik kesihatan to get refer letter to see eye specialist at HSI
